Ready, Set, Win!
This year, our Fall Fundraiser is a 50/50 Cash Raffle.  Tickets were sold August
30th-October 15th
.  All money raised during that time period will fund the PTA
budget (50%) and the Raffle Prize Pool (50%).  Each ticket purchased is an entry
to
win one of 21 cash prizes.

Direct Donation
Q.  How does the PTA raise money for both
Chesak and Martin Elementary Schools?
A.
 We hold fundraising events, such as our
Fall Fundraiser, Dine-and-Shares and
Cookbook Sales.  These fundraisers are our
primary source of income.  

Q.  What does the PTA do with the funds
raised?
A.
 The money raised by the PTA goes
towards valuable programs and services
supported in both schools, including field
trips, assemblies, family events, etc.

Q.  Why does the PTA have so many
fundraisers?
A.
We understand the need for diversity in our
fundraising.  We hold different fundraisers to
appeal to different personalities and budgets.

Q.  What about Spirit Wear?  Isn't this a
fundraiser?
A.
 This programs is not a fundraiser. The
PTA does not offer Spirit Wear to generate
funds, but because it promotes school spirit
and school pride.

Q.  Why should I donate or participate in
fundraisers for the PTA?
A.
 Donations and fundraising make it
possible for our PTA to continue its
programs, building upon the educational
experiences of our children. Fundraising
presents the opportunity for people that don't
feel comfortable with a cash donation, or
simply can't afford to donate, to contribute to
the PTA.

Q. What about Box Tops for Education and
Labels for Education?
A.
 The PTA does not collect these items,
however, the music department does.  You
can send Box Tops and Labels for Education
with your child to school.

Raffle FAQ:
Q.  How much are tickets?
A.
 1 ticket for $20 or 3 tickets for $50.  (Ticket sales ended October 15th)

Q.  Is there a limit to the number of tickets I can purchase?
A.
 There is no limit to the number of tickets you can purchase.

Q.  How many prizes do I have a chance of winning?
A.
 There will be a total of 21 prizes.  Three (3) prizes will be awarded each month
for 7 months.

Q.  If I win one of the monthly prizes, am I still eligible to win the other monthly
prizes?
A.
Once a ticket becomes a winner, it is no longer eligible to win in future monthly
prize drawings.  The only way to win in multiple months is to purchase multiple
tickets.  

Q.  When are the winners announced?
A.
 Monthly drawings will be held at the PTA General Membership Meetings (2nd
Thursday of each month), beginning November 2011-May 2012.  

Q. Do I need to be at the meeting to win?
A.
 While we encourage participation in our monthly meetings, winners do not
need to be present and will be notified by email or phone.

Q.  Do I have to have a child in either Chesak or Martin in order to participate?
A.  
Our Raffle is open to everyone, whether or not they have a child in one of our
schools.  

Q.  I don't want to participate in the raffle.  How can I still help?
A.
 Direct donations are always accepted.  We are a 501(c)(3) non-profit
organization and all direct donations are tax deductible.
A raffle is very easy for our families
to participate in.  Simply fill out the
Raffle Ticket Order Form, ask family
and friends to participate, send in
your order and then receive your
tickets.  It's that simple.  With a $20
donation to the PTA for a raffle
ticket, you have made a huge step
towards supporting the organization
that supports your children and staff.

We've tried other fundraisers in the
past and have not have the success
we need to continue funding our
programs, such as assemblies
and field trips.  
Why a Raffle?
1. RAFFLE TICKET PURCHASE IS NECESSARY TO ENTER OR WIN. The purchase price has
been set at $20.00 each or 3 for $50.

2. ELIGIBILITY: The Raffle is open only to legal residents of the 50 United States and the
District of Columbia, who are 18 years of age or older as of 08/26/2011. Parents, staff, PTA
members/officers and community members are eligible to enter the Raffle.

3. TIMING: Raffle money will be collected from 8/26/11 at 12:01 CST through 5:00 p.m. CST
10/15/11.  All money collected will be split 50% to fund the PTA and 50% to fund the monthly
raffle prize pool. Total amount collected will be posted on our website (http://www.cmpta158.
org) no later than Monday, October 17, along with monthly prize allocation amounts.

4. HOW TO ENTER: No ticket limit per person. Tickets are available for purchase through the
official order form.  To enter, visit http://www.cmpta158.org to download the official order
form.  Correctly complete the official order form and transmit as directed. On official order
form, all required information/fields must be fully and accurately completed or entry will be
voided. Refunds will not be issued.

5. PRIZE: The following prizes will be awarded each month November 2011-May 2012: a) 1st
prize 50% of monthly prize pool, b) 2nd prize 33% of monthly prize pool, c) 3rd prize 3rd 17%
of prize pool.  The prize is nontransferable and must be accepted as awarded. Proof of ID is
required to collect all prizes. Once a ticket has won in a month it will be discarded and will not
be used again. However if a person purchases more than one ticket, they are eligible to win in
multiple months with the other tickets still eligible in the raffle.  No substitution may be made.
Subject to the conditions herein, prize will be awarded. Winner is solely responsible for any
and all applicable federal, state, and local taxes (including where applicable sales tax) due for
an awarded prize.

6. NOTIFICATION: Winner(s) will be notified at the time of each drawing by phone or email.
Potential winner does not need to be present to win. Winners will also be posted on our website
within one week of the monthly drawing.

8. E-MAIL ADDRESS: In the event of a dispute regarding who submitted an online entry, the
entry will be deemed submitted by the natural person who is the authorized account holder of
the submitted e-mail address at the time of entry (proof thereof may be required of a winner).

9. RELEASE: By participating in the Raffle and/or accepting a prize, a participant/winner
agrees to defend, release and hold harmless the Chesak Martin PTA (Sponsor), and their
officers, committee chairs, members, representatives, and agents (collectively "Releasees")
from and against any actions, claims and/or liability for injury, death, loss or damage of any
kind (including any for the violation or infringement of any proprietary or personal right of any
individual or entity)("Liability") resulting in whole or in part, directly or indirectly, from
participant's participation in the Raffle, and/or participant's use, acceptance, or possession of a
Raffle prize, and/or participant's participation in a Raffle prize-related activity. By participating
in the Raffle and/or accepting a prize, a participant/winner consents to and gives the Sponsor
the right to interview, photograph/video a winner, and in perpetuity to freely and exclusively
use, publish, and display a winner's name, entry form, picture, portrait, likeness, voice, video,
statements, quotes, biographical information, and prize information (in whole or in part, as may
be adapted, edited, or modified, as solely determined by Sponsor) for advertising, trade,
publicity and promotional purposes in any media now known or hereafter discovered,
worldwide, and on the World Wide Web, without review, notification or approval, all without
additional consideration, unless prohibited by law.

10. CONDITIONS: Participants agree that by participating in the Raffle they release the
Releasees from any Liability in connection with: a) Entries that are ineligible as a result of
being lost, late, illegible, damaged, incomplete, stolen or misdirected; b) any typographical or
other human or technical errors in the offer or administration of the Raffle, including but not
limited to errors in advertising, these official rules, the selection and announcement of a winner
(s), or the distribution/awarding of a prize(s); c) any omission, interruption, deletion, defect,
delay, misdirection, damage, injury, technical malfunctions or traffic congestion on the
Internet/telephone network, or at any website, or any combination thereof; or d) acts of God,
natural disasters, or other force majeure matters out of Sponsor's control. Sponsor reserves the
right to modify these rules for clarification purposes without materially affecting the terms and
conditions of the Raffle. If for any reason the Raffle is not capable of running as planned as a
result of but not limited to an error, omission, defect, delay, misdirection, tampering,
unauthorized intervention, fraud, action of entrants, infection by a computer virus/worm,
technical failure, natural disaster, act of God, disruption of the public markets, act of terrorism,
war or any other cause which in the sole opinion of Sponsor corrupts or affects the
administration, security, fairness, integrity, or proper conduct of the Raffle, the Sponsor reserves
the right at its sole discretion to withdraw, cancel, terminate, modify or suspend the Raffle
without obligation or prior notice and in such event to determine the winner via random
drawing from among all valid entries received up to the time of such cancellation,
termination, modification or suspension. In such event, Releasees shall have no liability to any
participant who is disqualified due to such an action. Failure to enforce any term of these
official rules shall not constitute a waiver of that provision.

11. LAWS: Subject to all federal, state and local laws and regulations. Void where prohibited
or restricted.

12. WINNER'S LIST: The winner’s will be announced at each monthly PTA General
Membership Meeting (Nov-May) and will be posted at http://www.cmpta158.org within one
week of each drawing.  

13. SPONSOR: Proceeds benefit the Chesak Martin PTA, a 501(c)(3) non-profit organization.
